• Shown compassion, leadership, resilience, or other key values of this award

• Made a meaningful impact on the patient or family's cardiovascular and thoracic journey.

• Went that extra mile that made all the difference

Winning nominees will be invited to attend Heartbeat’s Autumn Ball at the Leonardo Royal Hotel, Southampton, on the 14th November to collect their award.

Nominations must be received by 26th September.

Judging to take place week commencing 5th October.
