• Shown compassion, leadership, resilience, or other key values of this award

• Made a meaningful impact on the patient or family's cardiovascular and thoracic journey.

• Went that extra mile that made all the difference

The winning nominees must be available on the Saturday 15th November to collect their award at Heartbeat’s Autumn Ball at the Leonardo Royal Hotel, Southampton.

Nominations must be received by 26th September.

Judging to take place week commencing 6th October.
